            S u b je c t:  ABSENCES  AND  TARDINESS  INCURRED  BY  MR.  JONARD  G.  ESCORPEZO

                I.     ABSENCES

                   Mr. Jonard G.  Escorpezo is constantly submitting his accom plished DTR every month w he
            he was transferred  in this cam pus from the Main  Campus.  Discrepancies occurred only when  h
            was  requested  or  tasked  by  the  form er  Campus  Executive  Director  Dr.  Ricardo  L.  Ildefonso
            anytim e to  render his driving  services during weekends or even  Holidays for an  Official  Busines
            to  Baguio  City,  Bontoc,  Kalinga,  Paracelis  and  to  some  M unicipalities  of the  Province  of  Ifuga
            with  verbal  agreem ent that  the  latter will  offset  his  overtim e  on  any  day  during  the  weekdays
            That  is the tim e around when  Mr.  Escorpezo fails to  subm it  his  DTR  on  tim e that causes  his  un
            updated  leave  ledger.  W hen  he  incurs absences  som etim es  started  on  May to July,  2018 whe
            the  tim e  that  he  went  to  hospital  for  a  check  up  because  of  his  poor  health  condition  and  hi
            Physician  advised  him  to  rest  for  at  least  THREE  (3)  months.  The  office  prepares  the  Back  t
            W ork  Order  for  him  dated  June  6,  2018  but  only  served  to  him  June  13,  2018.  Prior  to  that,
            asked  Mr.  Ronald  Aliguyon,  HR  staff  in  this  Campus,  to  som etim es  visit  Mr.  Escorpezo  in  hi
            residence  at  Santiago  City  to  know  his  present  condition  before  the  Back  to  W ork  O rder  wa
            served.  W e have  learned that he was  advised  to  have a  bed  rest for at least THREE  (3)  month
            as  stated  above  but  we  still  order  him  to  report  to  file  his  leave  of absences  and  bring  medica
            certificate as proof to justify his absences so that he will not be charged of Absence W ithout Officia
            Leave (AWOL).  Mr.  Escorpezo cam e despite his poor health condition to file his leave of absence
            and  medical certificate with  him.




                II.    ACCUM ULATED  LEAVE  CREDITS



               As  per  record  on  file  in  his  Leave  Ledger  Card,  the  Accum ulated  Leave  Credits  of  Mr.
            Escorpezo as of February,  2019 is shown  below with the following details to wit;

               VACATION  LEAVE                  -      57.298
               SICK LEAVE                       -      32.000
               TO TAL LEAVE  CREDITS            -      89.298


               III.    NON-SUBM ISSION  OF  DTR:

               Mr.  Escorpezo  is subm itting  his  DTR to the  Main  Cam pus that  is according  to  him  but  I  found
            out  that  based  on  his  leave  ledger  that  is  on  year  2015  when  Mrs.  Imelda  Guyon  is  still  the
            designated  Human  Resources  (HR).  W hen  Mrs.  Guyon  was  given  the  opportunity  to  get  an
            Instructor item  in  the  College  of Agriculture  and  Forestry,  that  is the tim e that when  the  Campus
            Director designated  me  as the  Cam pus  Human  Resources  (HR).  Mr.  Escorpezo  is  continuously
            submitting  his  DTR  on tim e  but he only failed  to subm it his  DTR started  on  November,  2016  and
            some  months  of  year  2017  that  causes  his  leave  credits  un-updated.  He  was  then  asked  and
            always  reminded  by the office to subm it those  but  he fails.  That according  to  him  he  is  not able
            to  subm it  his  DTR due to som e  lacking  docum ents  as attachm ents  until  such  tim e that he forgot
            to accom plished such.
