5. If you are on personal leave of absence, you must keep your supervisor informed of your status, and contact
the company at least two (2) weeks prior to your scheduled return to work.
6. Upon return from your personal leave of absence, every effort will be made to return you to your original
position. However, depending on individual circumstances and business conditions, reinstatement is not
guaranteed.
INSURANCE COVERAGE WHILE ON LEAVE
If you are covered by company insurance and currently paying for any insurance coverage, you must continue to
make such insurance payments during any unpaid leave of absence under the policy; failure to make such payments
may result in termination of your coverage. If you are receiving pay for part of the leave, your share of health
insurance premiums will be deducted from your pay consistent with normal payroll practices. If leave is unpaid, you
must make arrangements with the company to pay any premiums due while you are out on leave. In the event that
you elect not to return to work upon completion of an approved leave of absence, in some instances, the company
may recover from you the cost of payments made to maintain your health coverage. You will then receive COBRA
enrollment information via mail.
MILITARY DUTY LEAVE (USERRA)
Bunkhouse is committed to protecting the job rights of employees absent on military leave. In accordance with
federal and state law, it is the company’s policy that no employee or prospective employee will be subjected to
any form of discrimination on the basis of that person’s membership in or obligation to perform service for any
of the Uniformed Services of the United States. Specifically, no person will be denied employment, reemployment,
promotion, or other benefit of employment on the basis of such membership. Furthermore, no person will be
subjected to retaliation or adverse employment action because such person has exercised his or her rights
under USERRA or similar laws providing leave for military service. If you need military leave, please see your HR
representative for details.
WORKER’S COMPENSATION LEAVE OF ABSENCE
Workers’ Compensation Insurance protects you in the event of injury or illness resulting directly from work. When
a claim is approved, this insurance generally covers most medical bills, provides a statutory benefit payment that
varies by state, as well as a death benefit. Any benefit received from Workers’ Compensation Insurance is used as
an offset to any benefit payable from the company. You must notify your supervisor immediately if you are injured on
the job, no matter how minor the injury, so that you can receive treatment and benefits where appropriate. Failure to
notify the company of an injury could affect your right to receive benefits, and may result in disciplinary action, up to
and including termination of employment. All workers’ compensation leaves of absence that also qualify under the
FMLA will count against the twelve (12) week FMLA entitlement.
REQUESTING A LEAVE OF ABSENCE
All requests for a Leave of Absence should be personally submitted in writing to the main office. All Time-Off Request
Forms are available in the main office.
