job done. As an employee, you are required to report to work on time, to observe the limits for breaks and meal
periods, and to not leave work early without prior approval from your supervisor.

                ABSENTEEISM
An absence is any time you are scheduled to work and do not report to work or complete your scheduled shift. It
includes sick days, but does not include pre- approved paid time off, holidays, or leaves of absence. A partial day
absent is any day you work less than your scheduled shift and which the company does not consider to be a tardy.

                Reporting Procedure
In the case of an absence you must make direct contact with your supervisor at least four hours before your
scheduled shift and each subsequent day you do not report to work. It is your responsibility to personally make the
contact unless you are physically unable to do so. You must give the reason for your absence and the expected date
of return. Please note that reporting your absence does not necessarily excuse the absence; rather, it is a required
                courtesy.

In addition, failure to make direct contact with your supervisor prior to the beginning of your shift when you know you
will be more than thirty (30) minutes late will be considered an absence and counted toward excessive absenteeism.
“Call-outs” to co-workers are unacceptable. Failure to notify your supervisor will be considered a “No call/No show,”
unreported or unexcused absence. Any unreported or unexcused absence will result in corrective action up to and
                including termination.

If you are absent for two consecutive scheduled shifts without reporting to work or personally contacting your
supervisor, you will be considered to have voluntarily resigned without notice as of the end of your second day’s
shift. This is considered job abandonment.

Direct contact with your supervisor is defined as a text or phone call that has been returned or responded to. If
you are having trouble contacting your supervisor, then make direct contact with the MOD or your onsite HR
                representative.
                Excessive Absenteeism (Reported Absences)

Even if an absence is reported, you may be subject to corrective action if you miss work too often.
                Examples of excessive absenteeism include:
                   • 12 days absent, consecutive or not, in any 12-month period
                   • 3 separate absences in any 30-day period
                   • 5 separate absences in any 6-month period
                   • 10 or more separate absences in any 12-month period

For purposes of this policy, a “separate absence” means:
                   • Any full or partial day absent, or

                   • One or more consecutive full or partial days absent for the same reason. unless determined by the company
to be an abuse, time off for medical/dental appointments, school activities (for you or your children) or
personal business will not be counted as excessive absenteeism if it is approved by your supervisor prior to
the schedule being posted. However, this time off will still be documented as an absence. You may be asked
to find someone to help cover the shift you are missing.
