How frequently have you been to the hospital or ER in the past year? For what
problem? General Tips
If patient has difficulty ambulating consider asking: Since you’ve been in the hospital Wash your hands upon entering & leaving the pt’s room
have you been out of bed walking around? How far have you walked? Can you get If appropriate, sit by patient and appear relaxed
up to go the bathroom by yourself without any help? Ask clearly and concisely if patient would like you to assess them for homecare
services - Do not make any promises, set expectations accordingly
Pre-Hospitalization Home Management Do not disagree w/ pt based on what the chart states
How were you managing at home before you were hospitalized? Where you able to Remember physician Interns are not necessarily the most accurate in
move around, get to your MD appts, wash, clean, cook for yourself? How was that documentation
working for you? Were you able to go shopping alone or did you need assistance?
Have you ever had any problem taking the meds that were ordered by your doctor?
Have you been on a special diet? Have you had problems maintaining that diet at

Do you use a walker or a cane? Has anyone from Physical therapy come to visit you
while you were here? I’ll speak with your MD and see if he wants a Physical
Therapist to see you before you go home or once you are home to help you get
stronger (say as appropriate)
How are you planning on getting home from the hospital?
How are you going to get your medications? You need to have them in your home
the first time the nurse comes.
Closing Up
If pt is going home the day of interview: Have you received any d/c instructions or a
d/c medication list or prescriptions from your doctor or nurse yet? (If no and d/c is
imminent speak with hospital nurse)
