Outcome Assessment Information Set (OASIS)

What drives the HHRG score?
C = Clinical (1-3)
F = Functional (1-3)
S = Service Utilization (1-5)

Rehab Need based on HHRG

 C1, F1, S1 = High Functioning, Low Risk

 C2, F2, S1 = Moderate Functioning, Moderate Risk

 C3, F3, S3 = Low Functioning, High Risk

 C3, F3, S5 = Very Low Functioning, High Risk (a lot of
psycho-social issues)
