LPN –ROLES & RESPONSIBILITIES

 Provide care in accordance to an established Plan of Care (POC)
including:
 Communicating and documenting all:
o Observations
o Measurements and
o Care provided
o Include subjective and objective patient/family information
 Report changes in the patients’ condition to the RN/Case Manager
• LPNs are not allowed to accept verbal orders from the MD, or
change orders on the POC

 Always identify self and role to patient/family and MD

 Order medical supplies & equipment under direction of RN

RN OVERSIGHT

Serves as resource for the LPN

Determines level of cases to be seen by LPN based on Scope
of Practice

Reviews and communicates on cases being seen by LPN
