Case Manager Responsibilities

 Who can be assigned as Case Managers?
 Registered Nurses and Physical Therapist
 Scheduling and managing visits
 Payor knowledge and management
 Coordination and collaboration with other members of the
interdisciplinary team

 At SOC , field clinician/ Case Manager establishes visit
frequency –this can include subsequent visits, recerts,
planned d/c, and hha supervisory visits. It’s important that
the Case Manager understand the patient’s current payor
in order to determine which services can be provided.
