Case Manager Responsibilities

 Communicate and collaborate with family and caregivers

 Completion and oversight of required documentation within
time frame- e.g. SOC, Recert, & Discharge OASIS

 Knowledge of Clinical pathways

 Participation in team/branch meetings

Coordination of Infusion Cases

All VNSNY patient infusions are performed by outside infusion nurses from
vendors

Visits can be made by both CHHA and infusion nurse

VNSNY Case Manager should communicate with infusion nurses

The start of care date will be the first visit made by either VNSNY or infusion
nurses

VNSNY is not responsible for ordering infusion supplies on managed care cases

Prior to discharge from home care services, Case Manager must confirm that
infusion nurses are no longer providing care
