Documentation of Visit Treatment Time

Requirements for accurate documentation times:

Accurate documentation of • In-home visit time is recorded from the start of the visit until the
visit time on each home care clinician gets patient’s signature just prior to leaving the patient’s
visit home and signing the visit off as “Incomplete” or “Complete”.

What is Included in • Time, in the patient’s home, providing care, education and case
management, including completing documentation.
Treatment Time?
• Time, in the patient’s home, speaking by phone with the
patient’s physician (only include calls to MD).

• Time spent communicating with VNSNY team members,
What is Excluded from manager or related work areas such as Contract Administration.
Treatment Time? • Time spent documenting outside patient’s home (this
includes documentation entered in clinicians’ home or care, or in
a VNSNY office).
