Discharge- Type 9

 Discharge from Agency-No Discipline Providing
Care

 Issue Generic Notice at least 2 days prior to discharge or on
previous visit :
 This can be done with a scheduled buddy code ( ABN ) for
planned discharges

 Complete OASIS in the scheduled discharge note RN18 if visit made

 Complete RN 66 if discharging patient and no visit is made.

 Complete a DISCHARGE SUMMARY note

**If you are at patient’s home on routine visit and decision is made to discharge
patient scheduler needs to be notified to schedule a SN18 visit **

Discontinue Your Discipline Others Continuing To Provide Care
 Contact scheduler to schedule a

 Coordination Note/VO from MD to discontinue

 Schedule a buddy Code for HHCCN indicating a notice needs to be
delivered and issue HHCN

 Complete a Discharge Summary Note RN 19 note- this note does
not include an OASIS

Death At Home Type 8
 Used if patient expires in home or on way to ED
 RN 88 visit note needs to be scheduled (visit not required) 22
