Key Considerations
1. Review medications and update as needed. Remember the DUR!
2. Review Goals/Expected Outcomes; remove all non-applicable goals.
Most likely, most of the goals have been met.
3. Discharge Plan: has the plan changed??
4. Are Nursing and HHA visit frequencies appropriate? Remove any
treatments you will no longer be providing, such as wound care,
instructing on skin care, etc….
5. Have you discussed the treatment plan with the patient, MD, patient
caregiver and\or patient representative and CFM?
