Recert Process

The Recert process is to include more information be sent to the MD,
especially the following :

• Measurable goals,
• Hospitalization risk, and Intervention

All clinicians must:
• update their goals for the Recert POC.
• document in the COC note that:
 Goals have been adjusted.
 Recert was clinically justified as discussed with MD
and clinical manager.

Planning…..

A visit must be made to do a Recert.

Don’t wait until the last possible day to make your Recert visit!

Call the patients MD and obtain a verbal order for the Recert. Review
Level 1 and 2 DUR results w/MD, (even if you’ve done this when the case
was opened). Document this in a COC note.

Discuss w/all active disciplines their plans for continuing or d/c of services
(PT, OT, ST, MSW). Again, document all conversations in a coordination
note. You need to know what the plan for the patient is!
