There are several types of OASIS:

Patient/caregiver interview.
Physician.
Physician orders.
Referral information.
Current medication list.
Clinician Observation
Physical Assessment.
Clinical record

When completing an OASIS the clinician has several data sources that can be used to
collect information. Some specific questions will clearly state when there are specific
sources that she be considered and used when responding to a question. For example the
question for anxiety specifically indicates either reported or observed whereas the
Depression assessment is answered by only the patient.
