Recertification

Planning for a Recert
 Discuss with clinical review manager 10 days prior to Recert due (Day #50)

 Discuss with patient MD to confirm in agreement with POC and continued
services. Discuss discharge plan. Document in COC screen.

 Refer to Clinical Pathway for guidance.

 Discuss with patient and caregiver.

 Reassess patient treatment plans and goals.

Recertification

Recert Process

 The clinical field manager (CFM) will notify the COC of upcoming
Recerts by day # 50

 Care manager will contact all disciplines to confirm treatment plans

 Must get updated orders for all disciplines and HHA to recertify

 With MD approval, discontinue diagnoses that are no longer
applicable

 Re-sequence diagnoses you are no longer addressing.

 Need for continued care note will completed by the CFM

 The scheduler will schedule a visit for Recert.
