Resumption of Care (ROC)

 Resumption of Care Oasis

 Generated when a patient has been on a TIF and returns home
(ROC) within the 60 day cert period.

 Clinician will be scheduled for a RN 15 or RN 05 which will
include a “Resumption of Care Oasis Type 3”

 Must update POC

 Think of this Oasis and POC like a SOC

Resumption of care key points

 When patient is admitted to hospital a Transfer Inpatient Facility (TIF) is
completed

 If they are discharged within the 60 day cert period the services will be
resumed.

 A resumption of care (ROC) RN 15 needs be completed within 24-48
hours.

 A revisit must be made for a ROC.

 If completed between days 56-60 the ROC- RN 05 will take the place of
the Recert OASIS.
