 Generates when there has been a “Significant Change in Condition”
(SCIC)

 Usually when you add a diagnosis during the episode

 SN17 is the visit note for a SCIC episode

Discharge Notification

 When discharging the patient all of the following need to
be notified:

• patient,
• patient representative (if any),
• patient caregiver
• all MDs issuing orders for the POC
• provider expected to care for the patient after
discharge (if any).

 *** Conversations with MD are recordered in coordination notes***
