STEP BY STEP PROCEDURE

 On subsequent home visits, review with patient the pre-
pour process.

 Evaluate the ongoing effectiveness of pre-pouring:
• Do medications remain at the end of pre-pour period?
o How many?

• Are medications gone before the end of the pre-pour
period?

• What problems does the patient have adhering to the
medication schedule?

 Use of:
• Written reminders
• Medication calendar
• Telephone reminders, etc.
• Medication adherence contracts
• Dosing alarms using wristwatch, pagers, computers or
smart phone
• Pill organizers
