                              Key Points to Remember


                              No staff member may administer, pre-pour or supervise the
                               administration of any medication that is not clearly labeled.

                              The Home Health Aide/Home Attendant may assist the patient with
                               pre-poured oral medication administration or blister packs as outlined
                               in the Paraprofessional Plan of Care left in the patient’s home.


                              The nurse must be aware of medications that should not be pre-
                               poured, such as PRN pain medications or others which must be
                               stored in its own container (i.e., sublingual nitroglycerin).

                              Patient's physical or cognitive impairments must be clearly
                               documented in the patient record either in the initial assessment or
                               subsequent visit notes.
