2. Therapist contacts the Case Manager on the case, who will be completing the Recertification
visit (RN02 or PT02 if the PT is the COC), and alerts him/her that they will also be continuing
into the next cert period
3. Contact the MD to obtain orders for the next cert period
4. Inform the patient that the therapy will be continuing into the next certification and discuss the
discharge plan.
5. Document all of the above conversations in the Notes section of the Medical Records.
6. Therapists who will be continuing into the next certification period must schedule and perform a
PT/OT/ST 06 (Add-on Recert) visit. This visit will allow the clinician to document and justify
further need for services past the current certification period.
HOSPITALIZATIONS
If a patient is admitted to the hospital for at least 24 hours during an episode of care they will be
placed on hold until they return, a.k.a. Transfer to Inpatient Facility (TIF).
Procedure for TIF
1. Therapist is informed that the patient has been admitted to the hospital.
2. Confirm if you are the first person to find out. This may be done by asking the family, or
calling the Case Manger and other disciplines. If you are the first person to find out, you
must perform a PT/ OT/ ST 44 visit service code (Transfer to Inpatient Facility visit)
3. Therapist obtains information from patient/family member in order to complete the OASIS
a. Date of admission to hospital
b. Name of hospital
c. Reason for hospitalization
d. If Emergency care services were utilized (Hospital ER; Doctor’s office emergency
visit, clinic emergency)
e. Date of most recent home care visit by VNSNY
4. Therapist calls all team members and MD to inform them of patient’s hospitalization.
Confirm with COC that you are completing the TIF visit.
If there is an existing visit for the patient, the clinician can document an Unexpected Event, and
choosing Transfer to Inpatient Facility
