               DISCONTINUATION and DISCHARGE

               VNSNY  Rehab  Services  makes  a  distinction  between  the  terms  discontinuation  and  discharge.
               Discontinue is used when one discipline is ending their service but other VNSNY disciplines are
               continuing their care.  Discharge is used when the entire case is being discharged from VNSNY i.e. the
               last discipline from VNSNY is ending their care, there are no other active disciplines on the case, and
               they are closing the entire case to VNSNY.  The process is similar for both with some added steps when
               discharging an entire case from VNSNY.


               Criteria for Discontinuation of Therapy


               Patients will be discontinued from Rehabilitation services when any of the following conditions are
               met:
                   1.  Goals are met.
                   2.  Improvement with stabilization is attained.
                   3.  Patient requires a different Rehabilitation setting (i.e. outpatient therapy.)
                   4.  Patient demonstrates a lack of potential for rehabilitation
                   5.  Patient refuses further treatment.
                   6.  Patient relocates or is away on vacation.
                   7.  Skilled service is not required.
                   8.  Patient missed visits or is not found, per agency policy.
                   9.  Patient expired.
                   10. Physician recommends discharge.
