Call-in Procedure. In the event of unforeseen emergency/sick
        leaves, all employees, regardless of the shift, should notify by sending
        SMS/text the number provided by HRD to the following offices:


                      FOR HEALTH CONCERNS OR ILLNESS
                                   HRD Office
                                 Company Nurse


           FOR OTHER CONCERNS OR NON-HEALTH RELATED MATTERS
                                   HRD Office

                  CUT-OFF TIME FOR THE CALL-IN PROCEDURE
                                 7:00AM - 9:00AM





                       SECTION 5. LEAVE BENEFITS


        A.INCENTIVE LEAVE


               5.1. Regular employees with at least one (1) year of service will
        be granted a service incentive leave with pay of five (5) days per year. Any
        unused leave credits are convertible to its monetary equivalent to be given
        to the employee at the end of the year or separation from employment.

               5.2. An employee who wishes to take leave of absence must file
        before the HRD a duly accomplished form and with the approval of his/her
        supervisor, at least five (5) days before the intended date of leave.


               5.3. To ensure continuous and orderly operations, the Production
        Department Heads will have the exclusive prerogative to decide the sched-
        ule of employee’s incentive leaves. The Department Head or immediate
        superior concerned should see to it that proper turnover of functions to the
        person who will take the place of the on-leave
