employee. The turnover should be done prior to the effectivity of the said
        leave.


               5.4. An employee who fails to return to work immediately after the
        expiration of the approve leave shall be considered as an Absent Without
        Official Leave (AWOL), except where the said failure to return is due to
        fortuitous event.


               5.5. Those availing of the service incentive leave (SIL) by reason of
        illness shall be exempt from the five (5) days notification but shall be subject
        to the following limitations:

             i.  It must be certified by the Company Doctor
             ii.  It must be approved by the Department Heads and HRD

               5.6. An employee must observe the “Call-in Procedure” on the day
        that he/she calls in sick. Upon his return, he must file before the HRD a Sick
        Leave Form accompanied with a medical certificate stating that he / she is
        “FIT TO WORK”.


               5.7. No employee may return to active duty after his sick leave
        without qualified clearance issued by the Company Doctor, certifying there-
        in that the employee is capable to fully discharge his/her assigned duties
        without impairing his/her own and/or coworkers health and safety.



        B. MATERNITY LEAVE


               Section 5.8. Any pregnant employee who has rendered an aggre-
        gate service of at least six (6) months for the last twelve (12) months im-
        mediately preceding the expected date of delivery or complete abortion or
        miscarriage, is entitled to the applicable Maternity Leave with pay, subject
        to the guidelines set forth herein:
