c.  A qualified employee shall inform the Company at least one month
             from the time he learns of his legitimate wife’s pregnancy by filing an
             accomplished form and with the approval of his supervisor.
             d.  Said leave may be availed before, during, or after the delivery or
             any miscarriage as long as it does not exceed 60 days after such de-
             livery or miscarriage.
             e.  HRD must be notified of any changes in the schedule of the leave
             of absence, provided that the same is with the approval of his immedi-
             ate supervisor



        D. SOLO PARENT LEAVE


               5.10. Solo parents who are defined under the Solo Parents’ Wel-
        fare Act of 2000 may avail of the seven (7) days Solo Parent Leave with full
        pay, subject to the following conditions:


             a.  Rendered at least one year of service whether continuous or bro-
             ken;
             b.  The qualified employee must submit two (2) copies of Request for
             Leave of Absence Form duly signed and approved by the immediate
             Supervisor.
             c.  A certified true copy of the Solo Parent ID shall be attached to the
             Request for Leave of Absence Form and

             d.  The Request for Leave of Absence Form must be filed five (5) days
             before the date of leave unless the cause for filing is considered an
             emergency and/or under any of the following circumstances:


             i.  When a child gets ill;
             ii.  He/she needs to attend Parent-Teacher Association (PTA)
             meetings in school
             iii. For enrollment purposes; or
