General Medicine & Geriatrics Summary

PHC: Geriatric and Medicine Rehab for older patients who may have several health problems or an
unexplained loss of function and independence.
SJHC – There are five specialized general medicine units to consolidate care for general medicine
patients, to expedite admissions from the Emergency Department and to better integrate

interprofessional care for our general medicine patients. Each unit is a specialty cohort, combining
patients with Stroke, Nephrology, Oncology, and Cardio. Resp.
SMH - GIM provides specialized care for patients with diseases involving all organ systems, and those
who have undifferentiated or multi-system disease processes. It houses the specialized Acute Care of
the Elderly (ACE) and Stroke units.
