●  Change Status from “All” to “Failed” and then click Generate Fax Log
                   ●  The updated log will show any fax in the selected time frame that failed to send, which
                       Coordinators should “Re-fax”
                          ○  If a document has failed, verify the correct fax number

               Expired & Expiring Plans of Care: Displays Medicare patients who either need a re-evaluation
               and new Plan of Care (if still being treated) or need to be discharged (if no longer being treated)

               Ending Authorizations: Displays patients who have authorizations that will expire in the next 2
               weeks (Time Frame is automatically set to 2 Weeks, but can be manually adjusted)
                   ●  Review authorization and confirm the number of visits used and when auth expires
                   ●  Review patient with PT and determine the necessary course of action:
                          ○  Will the patient be ready to discharge by the end of authorization?
                          ○  If the patient will not be ready to discharge, an outcome measure and progress
                              note must be completed in order to resubmit for additional authorization.

               Expired Authorizations: Displays patients whose authorizations have expired within the last 2
               weeks
                   ●  If the patient is still being treated, additional authorization must be requested and may
                       need to be backdated in order to cover visits after the previous authorization expired. An
                       outcome measure and progress note must be completed in order to resubmit for
                       additional authorization.
