Self Pay Guidelines (PT & Performance Training)
Physical Therapy
Patients may be seen at a Self Pay rate for physical therapy appointments if they do not have
health insurance, or if they choose not to use their health insurance benefits. A standard patient
chart must still be created in WebPT, and Self Pay will be listed as their Insurance. The
following rates are to be collected:
● Initial Evaluation: $145
● Follow Up Visit: $95
● Post-surgical Follow Up Visit: $70
● Medicaid: $35 per appointment (including eval)
All payments for a Self Pay patient must be recorded in WebPT as Internal Payment, with the
description stating Self Pay.
Performance Training
R2P previously offered small-group and individual performance training sessions, but due to the
high volume of patients, the training is no longer offered (unless done so directly by a PT in very
rare instances). Any clients seen for performance training must have WebPT charts and will be
entered as Self Pay. Appointments are scheduled in WebPT as 1-hour appointments, unless
otherwise stated. Cancellation/No Show Policy still applies.
1. For new clients, create a New Patient chart in WebPT. Collect all of their information per
standard protocol.
○ Input Self Pay as their Insurance
○ Add Case and name it Performance Training
2. For previous clients/patients, create a new case in their chart named Performance
Training and input Self Pay for Insurance.
3. At their first session, the client must complete the Performance Training Specific Intake
on the iPad.
4. Cost per hour session: $145. Payments must be recorded in WebPT as Internal
Payment, with the description stating Performance Training.
○ Similar to a copay, payment will be collected at each session.
○ Previous training clients are grandfathered in under current payment amount.
