schedule basis (i.e., reducing the usual number of hours you work per workweek or
workday).
Intermittent leave may not be taken in increments of less than one hour.
Because the leave is unpaid, the Company will reduce your pay based on the amount of
time actually worked. Accordingly, only the amount of the leave actually taken will be
counted toward your twelve (12) week leave entitlement. If the intermittent or reduced
schedule leave is unpaid, your wages or salary will be reduced by the amount of the
leave taken.
In addition, while you are on an intermittent or reduced schedule leave, the Company
may temporarily transfer you to an available alternative position which better
accommodates your recurring leave and which has equivalent pay and benefits.
If you leave on an intermittent or reduced schedule basis, you must attempt to schedule
your leave in a manner that does not unduly disrupt the Company’s operations.
CALIFORNIA STATE DISABILITY INSURANCE BENEFITS
California based employees on medical disability leave for eight (8) or more consecutive
calendar days may be eligible for California State Disability Insurance (SDI) benefits.
Employees are responsible for applying for these benefits themselves and can obtain
forms from their healthcare provider. Employees receiving SDI will not be required to
use accrued vacation.
PAID FAMILY LEAVE BENEFITS
California based employees who suffer a wage loss in order to care for an ill child,
spouse, parent or registered domestic partner, or to bond with a new child may qualify
for “Paid Family Leave” benefits from the State of California. Paid Family Leave
provides benefits (partial wage replacement) during leave. It does not provide
additional leave, job protection or reinstatement rights beyond those provided by our
policies or by applicable state and federal law. Paid Family Leave is concurrent with
leave taken under those laws. Application forms are available in Human Resources.
PERSONAL LEAVES OF ABSENCE
Policy
Personal leaves of absence may be granted to regular full-time and part-time
employees who have completed their 90 day Introductory Period at the Company’s
discretion. Temporary employees are not eligible for personal leaves of absence. This
type of leave is without pay or benefits. A personal leave of absence may be
