Wellness Incentive Program







         We know how important personal health is to the overall quality of our lives. That is why we offer programs that support you and your wellbeing.

         If you are enrolled in the Basic HSA or Premier PPO HRA Medical Plan, you have the opportunity to earn Wellness Dollars.  ABC Company will
         deposit funds into your Health Savings Account (HSA)* or Health Reimbursement Account (HRA) if you complete the program defined below. The
         amount you can receive depends on the medical coverage category you elect for your medical insurance. See the below table for ABC Company’s
         contributions by coverage category.

         Your Wellness Incentive Program

         Earn a total of 100 Points by completing wellness activities through Health Advocate by April 30th, 2020 to receive ABC Company’s employer
         contribution. If you complete the 2019-2020 Wellness Incentive Program, contributions will be allocated into your HSA or HRA Account in June,
         2020. If you exceed 100 wellness points you will be eligible for additional prizes.

           Personal Health Profile (PHP) - 20 Points
                                                                 Biometric Screening or Physician Form - 30 Points
           Preventive Screening* - 30 Points (or two from the below list and receive 60

              •   Skin Cancer Screening                          Well-Being Activities 10-50 Points
              •   Vision Exam                                           Gym Usage, Volunteering, Blood Donation, Race/Marathon, Baby


        Example of how to earn 100 points: Complete the PHP, Get a Dental Cleaning and Biometric Screening, Run a 5K and Get a Flu Shot.

        Please note: if you become eligible for benefits after April 30, 2020 (Hired after February 29, 2020), you will not qualify to earn the wellness
        dollars until the next plan year.

        *You will be asked to verify your identity when you open an HSA Account because it is a personal bank account. An HSA Account must be established prior to funds being deposited into
        your HSA.  See page 9 for additional details.



           Accessing the Health Advocate Wellness Incentive Program
           Go to www.healthadvocate.com/members or download the app in the Apps Store or Google Play.
           •   Click on “Register Now”.  Enter the requested personal information below and then click Save and Continue.
                   •   First Name, Last Name, Date of Birth and Zip Code (Enter your information exactly as it is documented with your employer).
           •   The system will prompt you to create a unique username and password, provide an active email address and select a security
               question to register.
           •   You will receive an email message advising you to verify your email before logging in for the first time by clicking on Continue in the
               email.
