Bass is currently enrolled in the FULL HMO plan.
(FULL HMO plan benefits are changing this year)

Bass decides to make the move from the FULL HMO plan to the HSA PPO plan. Besides
saving money, there are other cool benefits that QSC offers to work with my HSA PPO
plan.

• QSC pays for setting up the HSA Bank account I will use with the HSA PPO plan

• I get a debit card from Discovery Benefits that I can use to access my money

• I can download the Discovery Benefits Mobile App and manage the money in my
account to pay for medical services, all from my smart phone

• I can download the My Cigna App to find out how much medical or dental services
will cost and shop for the best price. I can also shop for my prescriptions on Good
Rx or One Rx for the best cost for my prescription.

This is a lot to learn, but I am glad that I am in charge of how I get to to spend my money
on benefits costs!
