Employee Assistance Plan
If you or a family member needs assistance balancing life’s demands, or require help with personal or
family issues, you can contact the Employee Assistance Plan (EAP). You may contact the EAP directly 24
hours a day, seven days a week by calling 855-775-4357 or by emailing rsli@acieap.com.
ABC Company provides the EAP to all regular, full-time and part-time employees. Coverage begins on
date of hire. The entire cost of this program is paid by ABC Company. Covered benefits include short
term counseling (five consultations per incident per 12-month period) and referrals to help you deal with
a variety of issues. The services are provided by the Reliance Standard EAP.

Travel Assistance Plan
ABC Company provides you and your dependents with medical, travel, legal, and financial assistance
services when you travel internationally or more than 100 miles from home. Services are provided
through OnCall, an international travel assistance plan available through Reliance Standard Life.
Services are available 24 hours a day, seven days a week and include:
• Pre-travel information about visa, passport, and inoculation requirements
• Legal referrals
• Replacement of lost or stolen travel documents
• Telephone translation services
• Physician, hospital, and dental referrals and appointments
• Evacuation and repatriation
• Emergency/urgent message transmission
• Coordination of replacement for corrective lenses and medical devices
