•  To prevent a similar incident from happening in the future, it is important for Team
                       Members to report incidents right away, so we can identify its root cause.
                   •  Employees are required to report any workplace accidents or injuries within twenty-
                       four (24) hours – including any breaches of safety – and to promptly report any
                       unsafe equipment, working condition, process or procedure to a supervisor.
                   •  Failure to do so may result in a loss of benefits under the state workers’
                       compensation law.

                       Failure to abide by iFLY’s safety and accident rules may result in disciplinary action,
                       up to and including termination.

               Customer Incidents

                   •  Events causing or that have the potential to cause injury to any customer, guest,
                       visitor, vendor, contract service provider, or any person not employed by iFLY while
                       on premises shall be reported immediately to the Manager on Duty or a Supervisor.
                   •  If anyone defined as a customer has an injury or event:
                          o  Remain Calm and offer First Aid if trained
                          o  Call for help
                          o  Stay with the injured person until help arrives
                          o  The supervisor will report the incident electronically
                   •  Do not discuss the cause, liability, or blame for the injury/incident with the injured
                       individual or bystanders.

               Workers' Compensation Insurance
               To provide for payment of your medical expenses and for partial salary continuation in the
               event of a work-related accident or illness, you are covered by workers’ compensation
               insurance, provided by iFLY and based on state regulations. The amount of benefits payable,
               as well as the duration of payments, depends upon the nature of your injury or illness.
               However, all medical expenses incurred in connection with an on-the-job injury or illness
               and partial salary payments are paid in accordance with applicable state law. If you are
               injured or become ill on the job, you must immediately report the injury or illness to The
               People Team. This ensures that iFLY can help you obtain appropriate medical treatment.
               Your failure to follow this procedure may delay your benefits or may even jeopardize your
               receipt of benefits. Questions regarding workers’ compensation insurance should be
               directed to The People Team.
