Plan for Me is an online tool which helps you compare your options and estimate
your costs for the year, all based on your unique needs.
To Get Started
➢ Go to healthpartners.com/planforme.
➢ Enter 19886 for the group number, all for the site number and 01/01/2019 as
the effective date.
➢ Enter age and gender for all covered members under the plan.
➢ Pick from a list of common conditions and medical procedures you think
you might get.
➢ Plug in any medications you are taking.
➢ Check to see if your doctor is in-network.
➢ Print out a summary or email it to yourself.
