CLAIM
Each application for benefits requires the claimant to explain the reason for being out of work from his/her most
recent employer . After the claimant is determined monetarily eligible, two major factors determine whether benefit
payments will be made:
1 . The reason for separation from last employment .
2 . Maintaining continuing eligibility requirements .
A Notice to Employer Form: is mailed to the former employee’s (claimant) most recent employer to request
verification of the reason for unemployment . You will be asked to indicate the last day the employee worked, the
reason they became unemployed and information regarding any special payments made, such as unused PTO
pay (if applicable) . An investigation is initiated based on the information the claimant and GM provides . The
information you provide will come from eye witnesses and what has been documented by your management team
from the time the claimant was hired up to the final incident that caused separation .
CASEBUILDER
CaseBuilder is a web-based platform which allows GMs to gather and submit documents and details in a fast,
secure environment for all stages of the unemployment process . GMs are the unemployment contacts for each
restaurant .
As claims are received for each restaurant, Equifax will start sending requests for information through CaseBuilder .
GMs will then receive an email notification indicating work that requires attention .
Logging In
CaseBuilder is a secure system that requires a login for each user . There are two ways to access the system:
• Directly through the WebManager Page at https://ucm .talx .com
or
• Through the link in the CaseBuilder e-mail notification message .
In order to login through WebManager you will need three things:
• Your Company Code (Included in the registration e-mail)
• Your User ID (Set up to match your corporate email address)
• Your PIN or password (you will receive a temporary PIN code when you are registered to use WebManager .
Use this PIN code to set up a PIN that is easy to remember .
