EMPLOYEE AND GUEST INCIDENT INVESTIGATIONS


                 Employee Incident Investigation Procedures

                 All employee incidents are reported immediately to Medcor . Do not delay the reporting of an incident . A delay in
                 reporting has been proven to have a direct impact on overall recovery time and medical costs .

                 An Employee Incident Investigation will be completed at the restaurant where the incident occurred . Managers will
                 investigate all incidents resulting in an employee injury using the following investigation guidelines:

                   •   Review the equipment, operations, and processes to gain an understanding of the accident situation
                   •   Identify and interview each witness and any other person who might provide clues to the accident’s causes
                   •   Investigate casual conditions and unsafe acts; make conclusions based on existing facts
                   •   Provide recommendations for corrective actions; implement temporary control measures to prevent any further
                       injuries to employees
                   •   Indicate the need for additional or remedial safety training

                 The Employee Incident Investigation is an online form located on FOXCentral .

                 Incident Recordkeeping Procedures
                 The Safety Coordinator at the Big Kitchen will monitor online incident investigation and worker’s compensation
                 claim records . Any documentation the employee provides their manager regarding an injury should be kept in the
                 confidential employee file in the restaurant .
