2021 Benefits Guide
Required Documentation and 2021 Enrollment Process
Identification DAWSON’s Beneits Enrollment is conducted using the ADP/

and Social Security Numbers (when possible) to the
federal government for each individual enrolled for
medical coverage. You can upload your documentation
through ADP/WFN as you complete your enrollment
on the conirmation page.
Benefits Eligibility
Covering Yourself
You may enroll in the beneits program if you are a
regular full-time employee who is actively working a
minimum of 30 hours per week.
Covering Your Family Members
Eligible dependents generally include your legally
married spouse, and same or opposite sex domestic
partner and children up to age 26. Children may include
natural, adopted, step-children, or children obtained
through court-appointed legal guardianship.
If you cover a domestic partner, you must complete a
domestic partner afidavit that is available on ADP/
WFN as you make your enrollment elections. If your
domestic partner is not your dependent for tax purposes,
DAWSON will impute the value of the portion of the
premium DAWSON pays on behalf of your domestic
partner which is taxable. More details are provided in
the Domestic Partner Afidavit. You can also contact
