There is a single application form available on the HSE website. Applications can be made at any point

in the one year e.g. from 1 January until 31 December. Original receipts must be provided, and the
form should be submitted to your local HR/Medical Manpower department. Appeals of refusals
should be sought through your hospital CEO or general manager.
Payment should be received 60 days after application. If you require assistance securing your CME
