learning.
Assessment:
Skill-based errors are prevalent, suggesting insufficient driver and rider training and
competency validation programs;
Lack of situational awareness, decision-making errors, and complacency were the most
common factors;
Current risk management practices appear insufficient to prevent recurring incident types;
Occupational incidents highlight the need for strengthened safety protocols in work areas;
A critical need exists to cultivate a culture of reporting "near misses“; and
Routine violations (e.g., non-wearing of PPE) stem from a lack of self-discipline.
Recommendations:
Strengthening the Vehicle Accident Prevention Program and the Rider Safety Card System
Institutionalizing structured training and certification for PAFMV drivers and riders
Conducting scenario-based drills using CRM and non-technical skills
Enforcing MRM protocols in all operational areas
Enhancing hazard tracking and timely corrective actions
