02               MISHAP REPORT





             GROUND MISHAP REPORT


             A  total  of  44  ground  mishaps  were  recorded.  Of
             these:
                 73% involved vehicular
                 Notably, 38% were motorcycle-related
                 16% were personnel injuries
                 11% were workplace-related
             Compared to the first quarter:
                 Vehicular mishaps decreased by 18%
                 DUI-related cases declined from four to two
                 Personnel injuries dropped by 61%
                 However,  workplace  mishaps  increased  by
                  66%

             Most Class C incidents show non-catastrophic events   Overall,  the  total  ground  mishap  occurrence  trend
             but highlighted missed chances for prevention and   shows a 25% decrease from 1st  to 2nd Quarter 2025.

             Assessment:
              Skill-based  errors  are  prevalent,  suggesting  insufficient  driver  and  rider  training  and
                competency validation programs;
              Lack  of  situational  awareness,  decision-making  errors,  and  complacency  were  the  most
                common factors;
              Current risk management practices appear insufficient to prevent recurring incident types;
              Occupational incidents highlight the need for strengthened safety protocols in work areas;
              A critical need exists to cultivate a culture of reporting "near misses“; and
              Routine violations (e.g., non-wearing of PPE) stem from a lack of self-discipline.
             Recommendations:
              Strengthening the Vehicle Accident Prevention Program and the Rider Safety Card System
              Institutionalizing structured training and certification for PAFMV drivers and riders
              Conducting scenario-based drills using CRM and non-technical skills
              Enforcing MRM protocols in all operational areas
              Enhancing hazard tracking and timely corrective actions
