              This highlights the need to enforce firearms
                safety programs, enhance mental resilience
                training, and strengthen leadership engagement
                and accountability by reinforcing supervisors'
                roles in monitoring, mentoring, and supporting
                personnel.
             OVERALL ASSESSMENT
                 #1           The decline in mishaps this quarter is attributed to intensified safety
                              promotion efforts by the Command and PAF Units, coupled with leadership
                              commitment and proactive engagement of all personnel.
                              However, a high frequency of preventable incidents persists, indicating

                              systemic failures in current safety protocols, human error, poor situational
                              awareness, and organizational gaps.
                 #3           Integration of Mission Risk Management, Standardization and Evaluation
                              (Stan-Eval) activities, and sustained safety education and training are
