Life Insurance and Disability



























        Life and AD&D Insurance
        Guaranty Home Mortgage provides basic life and AD&D insurance for employees and offers voluntary insurance options
        for employees and their dependents.

        Basic Life and AD&D Insurance
        Life insurance is an important part of your financial wellbeing, especially if others depend on you for support. Guaranty
        Home Mortgage provides basic life and accidental death and dismemberment insurance at no cost equal to $50,000.
        Coverage is automatic; you do not need to enroll.

        Disability Insurance
        Disability insurance provides income replacement should you become disabled and unable to work due to a non-work-related
        illness or injury. Guaranty Home Mortgage provides you with Short and Long-Term Disability at no cost as shown below.
        Coverage for these two plans are automatic. You also have the option to purchase an additional Buy-Up Short-Term Disability
        plan if you choose.

                Coverage                                               Benefit

                                    ●  60% of your weekly salary, to a maximum of $500 per week for the first 12 weeks of a disability
         Short-Term Disability
                                    after the 7-day waiting period.
         Buy-Up Short-Term          ●  30% of your weekly salary, to a maximum of $1,000 per week for the first 12 weeks of a disability
         Disability                 after the 7-day waiting period.

                                    ●  60% of your base salary, to a maximum of $10,000 per month if you are disabled and are unable to
                                    work for more than 90 days.
         Long-Term Disability
                                    ●  Benefits are offset with other sources of income, such as Social Security and Workers’
                                    Compensation.





            Family Medical Leave Act (FMLA)
            If you have been with the company for 12 months, you may be eligible for up to 12 work weeks of unpaid leave per year
            under the Family and Medical Leave Act (FMLA). FMLA can be used for an illness of your own, care needed for a family
            member, care for a newborn and certain other medical needs.
