MEDICAL COVERAGE

GenesisCare offers three medical plans, all offered through Blue Cross Blue Shield of Florida.
– Value HDHP which has the lowest premiums and higher deductibles;
– Core HDHP which has mid-range premiums and deductibles; and the
– Enhanced PPO which has the highest premiums with lower in-network coinsurance,
copays and deductibles.
The chart on the following page allows you to compare key differences between each of the
three offerings.

PHARMACY COVERAGE

Pharmacy coverage is included when you enroll in a GenesisCare medical plan and is provided
by CVS/Caremark. The chart on the following page compares the differences in pharmacy
coverage between the three medical offerings.
Our pharmacy coverage includes the following prescription management programs:

PHARMACY COVERAGE HIGHLIGHTS
If your doctor prescribes a brand-name drug when equivalent generic drugs
are available, you will automatically receive an FDA-approved generic drug*
unless:
Mandatory
Generic – Your doctor writes “dispense as written” (DAW) on the prescription; or
– You request the brand-name drug at the time you fill your prescription
Substitution
If a generic is available, but you or your doctor request the brand-name drug,
you will pay the generic copayment PLUS the full difference in cost between
the brand-name drug and the generic equivalent.

If you take a maintenance medication on an ongoing basis for a condition like
high cholesterol or high blood pressure, you can save money when you have
Mail Order them delivered by mail, in 90-day supplies, from the CVS/Caremark Mail Order
prescriptions Pharmacy. CVS/Caremark will allow 2 "grace" refills at a retail pharmacy
for maintenance medications before you will need to switch to the Mail
Order Pharmacy
* When more than one generic drug is approved, CVS Caremark may fill your prescription with any approved
generic equivalent.
