Life and AD&D Insurance


         Basic Life & AD&D
         Life insurance is an important part of your financial security,  especially if others depend on you for support.  Accidental
         Death &  Dismemberment (AD&D) insurance provides additional coverage if your death or the loss of a body part or
         function is the result of an accident.

         The Company provides full-time, regular employees working 30 or more hours per week with Basic Life and AD&D
         Insurance after 60 days of employment.  This benefit is provided at no cost to the employee.  The  amount of coverage
         provided is equal to two times your annual base salary, up to a maximum benefit of $500,000.

         Voluntary Life
         You can elect additional coverage for yourself, your spouse and your dependent children with the following parameters.

               Employee:  One, two or three-times employee’s annual salary
               Spouse:      $5,000 – $50,000 in $5,000 increments
               Child:         $5,000 or $10,000 per child up to age 26

               Note:  Employee must elect supplemental coverage in order to elect spousal and/or child coverage. Spousal coverage is
               limited 50% of employee coverage.  Employee supplemental coverage is limited to $350,000.


         Disability Coverage

          Thrall Enterprises, Inc. provides eligible employees with disability income benefits at no cost after 60 days of employment.
          This benefit  provides you with income replacement should you become disabled  and unable to work due to a non-work-
          related illness orinjury.

          Short-Term Disability (STD):
          This benefit lasts up to a maximum of 26 weeks and is determined by length of service with the Company.  Salary
          continuation will be paid according to the following schedule:


                 Length of Service    Disability Benefit
                 0 – 6 months         No payment
                 6 months – 1 year    Up to 26 weeks at 60% of base salary
                 1 – 5 years          3 weeks at full salary, up to 23 additional weeks at 60%
                 5 years or more      6 weeks at full salary, up to 20 additional weeks at 60%


          Long-Term Disability (LTD):
          If you are unable to return to work after the 26 weeks of STD has expired, you may qualify for continued payment
          under a long-term disability policy provided by the Company and administered by Cigna.

          Qualified applicants may continue to receive up to 60% of their pre-disability monthly salary.  The length of LTD
          disability payment is determined by severity of illness or injury, age at the time of disability, normal retirement date
          and other factors.
