Dependent Eligibility



        Your dependents are eligible for benefits at the same time you become eligible. If you enroll, you can also
        enroll your eligible dependents. You will be required to provide your dependent’s Social Security number
        and documents of their eligibility.

        Eligible dependents include:
        • Your legal spouse
        • Children under the age of 26
        • Unmarried children incapable of self-care due to a qualifying disability

        Making Mid-year Changes



        Current IRS regulations limit changes in your coverage during the year. You may make changes when you
        experience a qualifying life event.

        Examples of IRS qualifying life events are:                IMPORTANT INFORMATION
        • Birth, adoption or placement for adoption                If you do not enroll in benefits or make changes
        • Marriage                                                 to your existing benefit elections within 31 days of
        • Change in child-care expenses                            the life event, you will not be able to enroll until the
        • Death of a dependent or spouse                           next Annual Enrollment period.
        • Dependent gains or loses eligibility
        • Divorce, annulment, or legal separation
        • Gain or loss of other employment/coverage by Team Member, spouse, or dependent
        • Gain or loss of eligible dependents through custody, court order, or guardianship
        • Team Member, spouse, or dependent becomes eligible for Medicare, Medicaid, or CHIP
        • Work schedule change (for example, Full-Time to Part-Time or Part-Time to Full-Time)

        If you experience a qualifying life event:
        •  You have 31 days from the event to enroll in benefits or make changes to your existing benefit elections.
        •  Only changes consistent with a qualified life event can be made.
        •  Follow the instructions for enrolling online and ensure you complete the process.
        •  Supporting documentation* of the life event is required and must be uploaded on the benefits website
          at the time of enrollment.
        •  Be sure to print out your confirmation page once you complete your enrollment, keep that confirmation
          page in a safe place so you can compare to your pay statement.
        •  Payroll deductions for the selected benefits will begin with the next pay period after the life event is
          validated by the HR Benefits Department. Any arrears owed will be collected in subsequent payroll
          deductions.

        *   Examples of supporting documentation include, but are not limited to, birth certificates, marriage
          certificates, divorce decrees, death certificate, etc.
