CVS Transform Diabetes Care Program

If you currently have or may be at risk for diabetes or hypertension, it can often be hard to manage the various aspects of
each disease. The CVS Transform Diabetes Care (TDC) program will help you to more easily manage your condition with
nurses available to help you along the way.

This program is available to support you whether you are in need of preventive actions or in need of active blood glucose
and blood pressure monitoring.

The program will provide:
• Two vouchers for a preventive monitoring visit at All medically enrolled members who are identified
a MinuteClinic (Note: MinuteClinic is not available for support based on their historical medical and
