Gift Yourself Well-ness

Gift Yourself Well-ness is our Company health and wellness mission. Good health and
wellness is one of the most important gifts that we can give ourselves. Together, we can
learn healthier habits to help us feel like our best selves.
One of the tools offered as part of our Gift Yourself Well-ness mission is a well-being
program offered through Virgin Pulse. If you are enrolled in a 1-800-FLOWERS.COM, Inc.
medical plan, you are eligible to participate in the well-being program offered through Virgin Pulse.
This program offers many ways to earn well-being dollars. Whether you want to eat healthier, lose weight, reduce stress, quit
tobacco products or something else, there are activities that can help you work toward achieving your goals.
How the Program Works

Company ever sees the results of activities you participate in, or any of your Protected Health Information (PHI). Rest assured, your
privacy is always protected, and the details of your participation are always completely confidential.
• Your information will never be sold or given to a third party for marketing purposes.

• Your personalized programs and information are available to you through a secure, password-protected website.
• Information will be used by the company to administer the program (for example, the payment of well-being dollars), but outside
of this purpose, your PHI, which includes your health assessment and biometric screening results, will not be shared with the
Company and may not be used for any other purpose.
