Voluntary Disability Insurance





       Disability insurance provides income replacement should you become disabled and unable to work due to a non-work-related illness or injury.

       We provide the option to purchase Short-Term and Long-Term Disability coverage through New York Life.

        Coverage                                     Benefit

                                                     • Choose $25 increments up to 60% income replacement with a maximum weekly benefit of $1,250
                                                     • Covers any non-work-related accident or illness
                                                     • Benefits begin after 14 days of disability
        Short-Term Disability (Salary Continuation)
                                                     • Benefit duration is 180 days including a 14-day waiting period
                                                     • Maternity is covered as any other illness
                                                     • Pre-existing conditions are covered after 12 months
                                                     • Choose $100 increments up to 60% income replacement with a maximum monthly benefit of $5,000
                                                     • Covers any non-work-related accident or illness
        Long-Term Disability                         • Benefits begin after 180 days of disability
                                                     • Benefits continue up to the Social Security Normal Retirement Age
                                                     • Pre-existing conditions are covered after 12 months

       Open Enrollment 2023-2024: Employees will be able to elect coverage with no additional medical questions. Pre-existing exclusions still apply.

         Family Medical Leave Act (FMLA)
         If you have been with the company for 12 months, you may be eligible for up to 12 work weeks of unpaid leave per year under the Family and Medical
         Leave Act (FMLA). FMLA can be used for an illness of your own, care needed for a family member, care for a newborn and certain other medical needs.
