O If you are also enrolled in the Guardian Dental plan,
you have an additional discount program through Davis Vision Contributions
Vision. This is provided to you at no additional cost.
O If you are also enrolled in the UHC Medical plan, you
have a vision screening benefit every 24 months.
O You will not receive a member ID card for the VSP VSP – Vision Plan
Vision plan. The provider will use your personal EE Only $8.68
