Basic Life and Accidental Death & Dismemberment Coverage

Offered through USAble Life, SEARCH Day pays for both of these benefits on your behalf. If you are working 30 or more hours a
week, you are eligible for these benefits. Life insurance is an important part of your financial security, especially if you support a
family. This benefit provides a disclosed amount to your beneficiary should something happen to you. Accidental Death & Disability
is very similar, as it provides a benefit to the same beneficiary if you are in a serious accident, or lose limbs or the ability to see.

Benefit 1st of the month following 60 days

Minimum Work Hours Required 30+ hours a week
Life Insurance Benefit
Guaranteed Issue Limit 2.5X Annual Salary up to max of $500,000, rounded up to next higher $1,000

Reductions $500,000
Life Insurance Benefits will reduce to:
AD&D Benefit
65% at age 65
50% at age 70
Max of $500,000
