Finding a Medical and Dental Provider
Step 1. Visit www.horizonblue.com/members.
Step 2. Click “Find a Doctor” on the very top of the page.
Step 3. Choose the service you are looking for, and then select the plan you are enrolled in.
Step 4. Input your zip code, and then click SEARCH.
Step 5. You can also refine your SEARCH by proximity, gender, and family, among other things.
