ELECTIONS AND CONTRIBUTIONS
You are not required to contribute towards the cost of this coverage.
BENEFITS
This program pays 60% of your pre-disability earnings (minimum monthly benefit of
$100; maximum monthly benefit of $12,500) once you have been unable to work in your
own occupation due to a disability for a period of 90 days. If your pre-disability annual
earnings are less than $90,000, you must be unable to work in any occupation for the
benefits to continue after 24 months of benefit payments. In general, benefits may
continue until age 65, but special rules apply. Benefits will be reduced to the extent you
earn certain other income while disabled.
