LIFE INSURANCE BENEFITS
ELIGIBILITY TO PARTICIPATE
Eligible Employees: All active L.L.C. owner-employees or non-seasonal employees of
the Nationals regularly working a minimum of 35 hours per week other than employees
covered under the Major League Baseball Players Benefit Plan or the Minor League
Baseball Health and Welfare Plan who have a life benefit of $80,000 or greater under one
of those plans (“Full-Time Employees”).
Child:
children (including your natural born children, adopted children, or step-children
living with you) under age 25; and
disabled dependent children age 25 and older, if covered by this program (or a prior
program) upon attainment of age 25.
Insurance Effective Date: January 1, 2015
New Hire Effective Date: Coverage will become effective on the first of the month
coinciding with, or next following, 30 consecutive days of regular Full-Time
employment.
CESSATION OF PARTICIPATION
The date on which you terminate employment or, if earlier in the case of additional
coverage that you elect, the last day of the period for which required contributions are
made.
ELECTIONS AND CONTRIBUTIONS
Basic coverage is provided at no cost to you. You are required to contribute towards the
cost of the additional coverage you elect at the rates established each year by the
Nationals.
BENEFITS
In general, this program pays your beneficiary two times your annual earnings, up to a
maximum of $80,000, upon your death. If you are covered under the Major League
